Just What Is Transitional Care? Transitional Care is the specialized care it takes—either on a temporary or continuing basis—to maximize your potential or to help you return home following a health event.

Let’s follow the Transitional Care rehabilitation program of one of our residents, Ruth Schmidt.

  1. Ruth had an elective hip replacement after a fall at her home. Following her time at the hospital, Ruth stayed at Shawneespring for physical and occupational therapy.

  2. The physical therapists worked with Ruth daily doing exercises to strengthen her leg muscles and help prepare her for once again putting weight on that leg.

  3. Occupational therapists taught her ways to put on shoes and socks, cook in the kitchen while using a walker, and how to get in and out of the shower.

  4. Ruth stayed at Shawneesprings for twenty days. When her family came to take her home, she’d gained the confidence to resume living on her own.
